Complex Case Manager III

Provide member-centric, evidence-based care for high-needs members to navigate the healthcare system and ensure safe transitions. Coordinate services across various settings and connect members with community resources to reduce the burden of illness. Requires an Associate's or Bachelor's degree in Nursing, a current RN license, and at least 5 years of clinical or acute care experience. Certified Case Manager (CCM) certification is preferred or must be obtained within two years.
